We have a four-bedroom bungalow in Milford Haven and a three bedroom flat in Pembroke Dock. Both properties are situated close to local amenities. Tenants will have their own bedroom. Support in Properties |
| Each property is staffed by experienced support workers for an average of 30 hours per week over a seven day period. Generally staff are on duty between 10am and 3pm, however, variations may occur due to external circumstance or at the tenants request. An emergency on-call telephone number is displayed at each property if assistance is required outside of normal hours |

Floating Support |
| Pembrokeshire Mind provides a team of experienced support workers working within the community offering minimal support to individuals within their own homes. Floating Support workers provides community support to four individuals within their own homes for between 20 to 35 hours per week, depending upon tenant support needs. An emergency on-call telephone number is available if assistance is required outside of normal hours. |

Keyworker |
A member of staff will be allocated to each tenant, as a ‘Keyworker’ the role of a keyworker is to work jointly with the tenants in the preparation of their independent living plan. Independent Living Plans (ILP) The aim of the ILP is to support the tenant in gaining the necessary skills, experiences and relationships for independent living and active participation within the community

| Support Provided Tenants might feel that they need support with some of the following: *Shopping *Cleaning *Cooking *Budgeting *Training/Education *Security of property *Arranging maintenance Reviews Regular reviews will take place to enable the tenant to discuss their progress and highlight any problem areas, thus ensuring that the facilities and support arrangements continues to be appropriate to the tenants needs. |
